Cancellation Policy

To avoid Cancellation fees, please give us 48-hour notice.

We will send you a reminder of your appointment but the patient is responsible to remember the time and date of the appointment. Please arrive on time to avoid not being seen and being charged for yours scheduled appointment.

No Show – You will be charged the FULL FEE.

Less than 24 hours notice of a cancellation – You will be charged the FULL FEE.

Between 24-48 hours notice of a cancellation – You will be charged ½ the scheduled FEE.

More than 48 hours notice of a cancellation – no charge.

Medication Refill Policy

We require that patients under medication management with us, who require prescription refills, must meet with us regularly or as recommended.

Please keep track of your prescription refill needs, as refills are provided during an appointment and are designed to last until the next recommended visit. If you find that a prescription refill is required please schedule a follow-up visit. Please see the refill police fee outside of medication management appointment. Refills will be handled in a case by case basis at the discretion of the provider
